Inquest into the Deaths of John Janssens, Michael Maukonen and William Swan Announced

Dr. Rick Mann, Regional Supervising Coroner for West Region, London Office, has  announced that a joint inquest will be held into the deaths of John Janssens, Michael Maukonen and William Swan.

Mr. Janssens, 73, died on January 15, 2016; Mr. Maukonen, 19, died on June 28, 2016; and Mr. Swan, 56, died on May 16, 2017, from injuries sustained from a fall while working at various roof construction projects. An inquest is mandatory under the Coroners Act.  

The inquest will examine the circumstances surrounding their deaths. The jury may make recommendations aimed at preventing future deaths.

Details regarding the date and location will be provided at a later date when the information becomes available. Dr. Elizabeth Urbantke will preside as inquest coroner. Gideon Bloch will be counsel to the coroner.
